Are you sure you're getting this with 2.1.111? The apparent cause of this
problem was fixed recently, in 2.1.109 IIRC.
It's possible that there are other causes, so it would be helpful if you
could try running the diagnostic patch Stephen Tweedie posted to the list a
while back. If you can't find it in a list archive, let me know and I'll dig
it up.
