If I recall correctly, the big move to uaccess.h happened in v2.1.6.
I think segment.h was still whole in v2.1.4.

Note: The other major change, elimination of verify_area()
was made in v2.1.2 (Oct 8). Linus, Alan, and others,
discussed it quite a bit. See the linux-kernel archives:
