[PATCH 075/114] x86: remove mach_apic.h

From: Ingo Molnar
Date: Wed Jan 28 2009 - 19:13:25 EST

Spread mach_apic.h definitions into genapic.h. (with some knock-on effects
on smp.h and apic.h.)
