Corrections to MAINTAINERS file

Hi Alan.

Enclosed is a patch to the MAINTAINERS file distributed with the
2.2.15 kernel source tree that consolidates all changes to date
that have resulted from my test message.

I haven't looked at the latest 2.3.99-preX version yet, but the
patch should apply reasonably cleanly against the 2.2.16 version
that I'm downloading at the moment...
