cdrom loosing files

i'm getting inconsistent results when accessing cds containing lots
of files. this happens with a plextor ultraplex as well as with a
teac cdr55s. a raw cd image mounted via /dev/loop exposes the same
behavior. no error messages are showing up. the cds are plain
iso9660.
