(Please note that we were getting these problems with 2.2.1 and moved to
2.2.1-ac5 to see if that fixed the problem. It didn't and in fact caused
one machine to lose all data on /)

I have NEVER seen apache zombie out like this. If it's not NFS,
then what is it? (nph-rage is a CGI which httpd is calling. touch is
touch. It is being called by nph-rage. The files being touched are on
NFS.)
