Module counts and the MD driver

I just noticed, rather painfully I'll have to admit (i.e. BOOM),
that the MD driver doesn't increase the module usage counts of
the block devices it holds open.

This means that I can repartition the disk even though there are partitions
in use by the MD driver, and (worse) if the block device is a module it can
be auto-unloaded while in use. Oops, indeed.

(This is 2.1.127 pre-3.)
