Data point:

Saw this too when shutting down what I think was one of the 115 pre-patch
kernels. I'm sorry I didn't get any details but there was nothing in
syslog and I'd booted into 115 release which had wiped the old kernel. I
haven't seen it again and have shutdown about 4 or 5 times since then.
