Ditto here.

But I have a script which then loops and tries again,
and always succeeds on second attempt.

In summary: run chat script to connect, run pppd, it dies: SIGHUP
loop again with chat connnect, run pppd, it works.

Ever since 2.1.9x, where x is somewhere in the high 90's.

Could be a ttyS problem.
