1.3.93, loop better but still broke

In 1.3.93, loop (use a file for a filesystem) works better, but still locks
up.
Before 91, it would lock up in less than a minute.
The fix for plug (in ll_rw_blk) (in 1.3.91) improved things, it would run
for several minutes before lockup.
I made another fix to 1.3.93, and it would stay up for tens of minutes.
I'm not going to submit that patch, because the darned thing is STILL not
working reliably!
It seems that the bug/deadlock is still there, and that these "fixes" merely
mask it (and make it even harder to find).
