Thanks for the help, I can confirm that with Herbet's patch applied I am now booting fine on Debian using

CONFIG_SCSI=Y
CONFIG_ATA=M
CONFIG_BLK_DEV_SD=M

on both an x86-64 (AMD Athlon64) machine with SATA boot disk and an i386 (Pentium 4) machine with PATA boot disk.
