[PATCH] ibmvscsi: avoid unnecessary use of kzalloc_pool

[Sage Weil]

The allocated struct is manually zeroed after allocation, so avoid using
the (broken) kzalloc mempool (which does not re-zero previously used items
when they are returned to the pool).
