[PATCH 1/2] bio: define __BIOVEC_PHYS_MERGEABLE

Define __BIOVEC_PHYS_MERGEABLE as the default implementation of
BIOVEC_PHYS_MERGEABLE, so that its available for reuse within an
arch-specific definition of BIOVEC_PHYS_MERGEABLE.
