[PATCH 4/4] x86: remove ifdef CONFIG_SWIOTLB in pci-dma.c

As other IOMMUs do, this puts dummy pci_swiotlb_init() in swiotlb.h
and remove ifdef CONFIG_SWIOTLB in pci-dma.c.
