[PATCH] uio: User IRQ Mode

This patch adds a "User IRQ Mode" to UIO. In this mode the user space driver
is responsible for acknowledging and re-enabling the interrupt.
Shared interrupts are not supported by this mode.

- flags should be unsigned long
