[PATCH 12/15] dm: add exports

Export core device-mapper functions for manipulating mapped devices
and their tables and move them to <linux/device-mapper.h>.

Protect the contents of device-mapper.h with ifdef __KERNEL__ FWIW,
and throw in a few formatting clean-ups and extra comments.
