It looks like bio->bi_bdev->bd_inode is 0xffffffff in generic_make_request().

(Please enable CONFIG_KALLSYMS: it gives better traces and you don't need
ksymoops).

The fact that this happens when you're doing something which thousands
of other people do makes me wonder. Is the oops always the same? Are
you sure the hardware is good?
