[patch] remove old-style timers

Migrates all users of 'struct timer_struct' over to 'struct timer_list'
and removes the old-style timers from the kernel.

Everything has been compiled except for

arch/ppc/8260_io/uart.c
arch/ppc/8xx_io/uart.c
arch/mips/baget/vacserial.c
drivers/acorn/block/fd1772.c
drivers/sbus/char/sab82532.c
drivers/sbus/char/zs.c

Everything that could be tested has been. Which was not much.

BTW: drivers/cdrom/mcd.c and drivers/cdrom/mcdx.c are broken: their
'struct cdrom_device_info' tables have not been supplied with a
devfs_handle_t.
