Patch shm against pre2-5

Hi Alan and Linus,

The attached patch fixes the following problems:

- negative dentries for IPC objects prevent shmat on reused shm ids.
- deadlock in shmctl(IPC_RMID) on the shm semaphore
- give the error of do_unlink to the caller if done directly instead
  of a printk
- reestablish meaning of shm_perm.mode | SHM_DEST so ipcs will report
  destroyed segments correctly.
- the callers of newseg_alloc/shm_alloc did not interpret the return
  code right in some places.
