Added the device name prefix to /proc/mounts and fixed a few minor bugs.

I'm having second thoughts about /proc/mount/*, because of its potential
security problems. Making it technically secure should be easy (add a
capability to access /proc/mount (off by default), and another one
authorizing a process to enable the former (on by default)), but it
would be quite confusing, and therefore de facto insecure.

Anyway, besides the few quota printks, I've pretty much exhausted my to
do list for this patch. Comments ?
